Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. To apply online or obtain an application for license visit https://www2.myfloridalicense.com/hotels-restaurants/ Observed establishment operating without a license, serving about 5 customers at time of inspection. Conducted licensing inspection and provided temporary license to operator.

Observed Handwash sink replaced by dump sink. According to plans sink was approved as a hand sink. Consulted plan review and who advised that establishment must operate according to approved plans. Operator agreed to change dump sink back to hand sink and add slash guard for food protection.
