Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed three compartment sink with quaternary ammonium solution testing at 0ppm. Person in charge added more quaternary ammonium tablets.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken stored over raw comminuted beef in walk-in cooler. Person in charge rearranged food items to proper storage levels.

Cutting board has cut marks and is no longer cleanable. Observed cutting board with grooves and no longer smooth and easily cleanable stored on flip top cooler on cook line. Flip top cooler not currently in use.
No handwashing sign provided at a hand sink used by food employees. Observed no handwash sign at hand wash sink by soda tower in server station. Person in charge posted hand wash sign.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed mold like substance build up on under side of chest reach in freezer lid.
Old labels stuck to food containers after cleaning. Observed old labels stuck to clean container lids on food item containers in heater unit.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed threaded hose bibb missing a vacuum breaker in laundry room.
Handwash sink missing in warewashing or food preparation area. Observed missing hand wash sink near ice machine in center beverage area. Person in charge had sink and accessible water and sink was installed by end of inspection.
