Raw animal food stored over/not properly separated from ready-to-eat food. Observed one container of raw ground beef stored on top of one container of cooked beef inside upright cooler located in kitchen.
Rodent activity present as evidenced by rodent droppings found. Observed total of thirty-six rodent droppings all on second floor dry storage in the following areas: ten on floor behind brick chimney, eight on top of cardboard box of carryout platters beside chimney, two in middle of floor from top of staircase leading to chimney, six on floor around HVAC unit, two on wall ledge leading up staircase from first to second floor, eight in window seal at top of staircase.

Ceiling/ceiling tile shows damage or is in disrepair. Observed hole/unfinished ceiling above chimney on second floor.

Both items in walk-in cooler are cook/cool 3.5/hours.
