Food stored on floor. Observed container lettuce and and salsa stored on floor inside walk in cooler.
Operator does not permit customers to use bathroom. Observed "out of order" sign on women and men's bathroom door. Women's bathroom works, but the men's does not. I informed operator if the bathroom in operating then the sign would need to be removed. Operator removed sign.
