Server stated they were brought from the walk-in cooler and placed on the counter at 8am this morning. Employee moved the butter into a freezer to rapidly cool to 41F.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Flat of raw shell eggs stored at cookline at room temperature. Employee at cookline stated they have been held at room temperature for approximately one hour and voluntarily discarded them during this inspection.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm). Employee changed sanitizer bucket and primed machine, still 0ppm. I manager had an employee begin setting up the three compartment sink. Inspector instructed manager to discontinue the use of the dish machine for sanitizing dishes and to use the three-compartment sink for sanitizing dishes until the dish machine can be repaired.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee at cookline cracked raw shell eggs with gloved hand, changed gloves, then began handling foam takeout containers without first washing hands. Inspector had employee stop and wash hands before continuing to work at cookline.

Manager stated the butter packets have been on the counter at room temperature for approximately one hour. Manager placed the butter packets into the freezer to cool to 41F.

Manager stated the potatoes have been on the counter for approximately one hour. Inspector instructed manager to reheat the potatoes to 165F, then continue to hold hot at 135F or above or cold at 41F or below. Inside refrigerated make-table in server station: potato salad (48F - Cold Holding). Potato salad stored in bowl above safe-load line on top of another pan inside make-table. Employee stated the potato salad was only stored there for approximately five minutes, then voluntarily discarded the potato salad.

Manager moved raw beef liver to the bottom shelf during this inspection.
Roach activity present as evidenced by live roaches found. One live roach on floor in kitchen outside of dry storage room containing onions and potatoes. Manager killed and discarded roach during this inspection. One live roach on leg of prep table in kitchen near back door.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In steam table at cookline: gravy (109F - Hot Holding). Manager began reheating the gravy to 165F and turned up the temperature at the steam table.
