Hole in or other damage to wall. Hole in wall near back door with pest trap and dead roaches.

Single-use gloves not changed as needed employee had cut finger with band aid and no gloves while preparing food with utensils. Educated she needed to wash her hands, put fresh bandage on and wear gloves even with utensils.
