Food-contact surface soiled with food debris, mold-like substance or slime. There is a black mold like substance inside the nozzle of the soda gun nearest the entrance to the bar. Operator replaced the dirty nozzle with a clean one.
No paper towels or mechanical hand drying device provided at handwash sink. There are no paper towels at the hand washing sink. Operator supplied paper towels.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Per operator all employee health reporting agreements are locked in the chef's office.
Operator is doing a special process, conducting reduced oxygen packaging, cooking time/temperature control for safety food to a time and temperature combination lesser than those in the Food Code or operating a molluscan shellfish tank for human consumption without a HACCP plan approved by the Division of Hotels and Restaurants. There are multiple packages of reduced oxygen packaged chicken dated 10/19 in the reach in cooler next to the ice machine.
