Employee washed hands with cold water. Observed employee wash hands in cold water. The hand wash sink employee washed hands at did not have hot water turned on when he washed hands.Discussed with person in charge and she discussed with employee.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on reach-in cooler at pizza make station and gasket on walk-in-cooler soiled. Also, hood area above conveyer oven soiled.
Water leaking from pipe and/or faucet/handle. Observed faucet at triple sink leaking. Person in charge stated they are waiting on plumber to repair.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket (Quaternary 0ppm) at front service line. Person in charge emptied and remade solution and retested at 200ppm.
A return visit. The problems from the previous inspection had been dealt with.

Operator unable to provide proof of employee training during this inspection.

Equipment in poor repair. Gasket on reach-in cooler at pizza make station torn. Also, gasket on walk-in-cooler torn. Operator stated they have gaskets on order. Also, multiple plastic lids covering foods at top of flip top reach-in cooler on cook line cracked.

Exterior door has a gap at the threshold that opens to the outside. Gap at base of rear door. (Daylight visible ).
