Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing at the end of the splitter for mop sink faucet where hose could be attached.
Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Men's restroom door not fully self-closing/tight-fitting.
Clean utensils stored between equipment and wall. At end of cook line, bread knife stored between flip top reach in cooler and wall. Employee removed knife from between wall and cooler and placed on cutting board.

Dead roaches on premises. Observed 1 dead roach on floor in front of pizza boxing station. Employee cleaned up dead roach and discarded.
Equipment in poor repair. Torn gaskets on both reach in coolers on cook line and stand up reach in freezer in back hallway.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Portable fan on floor near pizza boxing station soiled with dust. Also, multiple gaskets soiled with food debris.
Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler on metal rack along back wall, unwashed produce (lettuce, green bell peppers and tomatoes) stored about containers of parm cheese, romesan cheese and ready to eat chicken.

Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Manager unable to provide training for Amber, Javonte, Isiah, Javion and Lila who have been employed for over 60 days.
Equipment and utensils not properly air-dried - wet nesting. In back room on shelf by triple sink, plastic pans stacked before properly air drying.
Equipment in poor repair. Multiple gaskets on both reach in coolers on make line have torn gaskets.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of bottom pizza oven has food debris on conveyor belt.

Single-service articles not stored inverted or protected from contamination. On make line, shelf above reach in cooler single service to go bowls not inverted. Manager flipped containers over to be stored properly.

Employee in prep area handled personal cell phone and immediately touched prep table, handle clean equipment without washing hands. Inspector coached employee on proper hand washing procedures.
Toxic substance/chemical improperly stored. Plastic bottle of detergent stored on top shelf in dry storage area with/above clean pans. Bottle moved and stored correctly.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Missing at the end of the splitter for mop sink faucet where no hose is attached. Manager began removing hose connected to chemical dispenser to obtain breaker on opposite side.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee cell phone on prep table in back kitchen. Phone moved and stored correctly.
Opened employee beverage container in a cold holding unit with food to be served to customers. Multiple opened employee drinks in plastic container in walk in cooler. Drinks removed.
