No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Per Operator, employees have been signing a company-specific health agreement form and they send the signed page to the Corporate office. Operator on duty provided three copies that were faxed from Corporate during the visit. Per discussion, the remaining copies will be obtained from Corporate and maintained on site.
