Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker present at top of splitter, explained to operator it needs to be at end of splitter with black hose attached.
Employee with no hair restraint while engaging in food preparation. Observed employee engaging in food preparation with out hair restraint.
Food storage container/container lid cracked or broken. Observed several plastic lids cracked, in use.
Opened employee beverage container in a cold holding unit with food to be served to customers. Observed two chocolate shakes in reach in freezer by back door exit. Operator placed in container and labeled for employees.
