Hotel and Restaurant license decal not displayed on mobile food dispensing vehicle. No decal affixed to exterior of MFDV. Operator stated she never received it from the state. I advised operator to call Tallahassee and provided phone # to operator.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. License number not affixed to exterior of truck.

Raw animal food stored over/not properly separated from ready-to-eat food. Raw octopus stored above ready to eat hot dogs and cooked mussels in reach-in cooler near entry door. Operator removed octopus to properly store.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. shredded mozzarella cheese, hot dogs (51F - Cold Holding) in single door reach-in cooler next to MFDV entry door. Operator stated she had door opened and closed a lot while prepping and serving lunch. Operator removed some juices from cooler to let more air flow in cooler.
Employee washed hands in a sink other than an approved handwash sink. Employee washed hands in triple sink because hand wash sink was full of items. Discussed with operator that hand washing to be done in designated hand wash sink.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board at reach-in cooler across from stove soiled/ stained. Also, portable cutting board next to stove soiled/ stained.
