Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken stored over raw cod in the reach in cooler at the end of the cook line. The operator properly stored the raw foods.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed hummus (57F - Cold Holding) in the reach in cooler at the salad/dessert station. The operator stated the hummus had been held for approximately 2 hours. The operator voluntarily discarded the hummus.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no sink and surface sanitizer test kit.
Carbon dioxide tank not adequately secured.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed multiple employees engaging in food preparation wearing watches.

Warewashing sanitizing solution exceeding the maximum concentration allowed. 3 compartment sink, sanitizer basin and sanitizer bucket solution above 500ppm. Operator notified of toxic levels and disposed of all sanitizer solution in service. Operator will use test strips to monitor sanitizer for safe dilution rate.
No soap provided at handwash sink. Handwashing sink at outside bar. Operator replaced soap.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice bin in server station slimy/moldy. Operator notified and made steps to clean and sanitize ice bin.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Both dish machines at outside bar not sanitizing properly. Operator notified and discontinued use of both machines, setting up 3 compartment sink with sanitizer.
