Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized: Three compartment sink quatrenary amonium sanitizer tested to 0ppm. Employee drained the sink and put new sanitizer in. Retested to 200ppm.

Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again: Pan of octopus in ziploc bags in walk-in cooler with a date of 12/09. However, the product is still frozen as evidenced by ice crystals in the packaging. Employee believes the octopus was removed from the walk-in freezer on 12/17 in preparation for use today. Explained to employee that it needs to be date marked for the date it was removed from the walk-in freezer for tracking purposes.
