Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Hot dogs held on flat grill at 105F/114F. Operator voluntarily discarded hot dogs.
Accumulation of lime scale on the inside of the dishmachine. Build up on interior top of machine.
Floor soiled/has accumulation of debris. Floor in hallway next to mop sink is soiled.
No Heimlich maneuver/choking sign posted. No choking poster. Provided DBPR Form HR 5030-105 and operator posted sign during inspection.

In double door reach-in freezer at prep area, Raw hamburger in pan stored over ham in zipper bag and carton of egg white. Manager moved items placing egg whites and ham over the hamburger and segregating the items.
Stop Sale issued on time/temperature control for safety food due to temperature abuse Milk 50F, 50F( cold holding ) in reach in cooler in front bar area manager stated milk was in cooler this morning, has been in cooler overnight.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Mill 50F, 50F( cold holding ) in reach in cooler in front bar area manager stated milk was in cooler this morning, has been in cooler overnight.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. No vaccum breaker on hose adapter on triple sink faucet, manager removed adapter Bibb from triple sink faucet.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Hand wash sink did not reach a temp above 80F. Manager called maintenance they repaired it. Water reached a temperature of 102F
Dead roaches on premises. 2 dead roaches in corner of walk in cooler. Manager cleaned and discarded.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee watch on shelf above food prep area on cook line. Manager moved watch to a designated area
Garbage on the ground and/or pad around dumpster. Excessive garbage on ground in the back of the trash can area.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Utensils on cook line sitting in water at only 126F. Employee moved container to heat on flat top, second temperature above 135f.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Inside of oven on cook line Is heavily soiled with food debris.
Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Shelf over triple sink covered in lining that was tearing and not easily cleanable. Shelf over wash station covered in lining that was tearing and not easily cleanable. Manager removed lining from both shelves.
Opened employee beverage container in a cold holding unit with food to be served to customers. Employee water bottle found in reach in freezer next to prep area. Manager through water bottle away.
Single-service articles improperly stored. Box of single service to go boxes was stored In a box on the floor, at back door area.
Use of cooking equipment producing grease laden vapors/smoke with no hood suppression system installed. Notified Fire AHJ. For reporting purposes only. Fryer that was hot and active found on prep table by triple sink. not under hood suppression system. Manager moved fryer under hood suppression system.

No handwashing sign provided at a hand sink used by food employees. Missing at hand washing sinks in both men's and women's restrooms in dining area. Chef states he has signs and will post.
