Employee stated it may have been out briefly and then stored near door being opened. Employee will keep walk-in-cooler door closed for butter to chill. Also, butter (76F - Cold Holding) at back front counter next to sandwich press. Employee stated butter has been out since they opened at 11am. Operator called to say it should be a 4 hour label but no label and no paperwork provided. Distributed TPHC paperwork to operator during this inspection.

Employee removed paper boats to properly store. Also, sealed boxes of wrapped straws and food paper bags stored on floor in storage area. Employee removed from floor to properly store.

Operator stated items were placed in front display cooler about an hour prior. Operator removed and placed in nearby reach-in cooler to chill.

Operator stated she cut lettuce at 11am. Observed salad cooler missing curtain/ door allowing cold air to escape from cooler. Operator placed cooling cut lettuce on ice to chill.

Employee removed lettuce from display cooler place in walk-in cooler and called maintenance for cooler.
