Dead roaches on premises. Observed two dead roaches on floor in back kitchen area. Pointed out to person in charge. Person in charge swept up dead roach.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed mini refrigerator gaskets soiled with food debris. Observed non food contact portions of red food mixer soiled with food debris.

Stop Sale issued due to food not being in a wholesome, sound condition. Observed 6 buckets of ice cream left inside walk-in freezer that has not been functional for months per person in charge. Ice cream has flys and visible mold. Walk-in freezer is scheduled for be removed from establishment.
Dead roaches on premises. Observed approximately 5 dead roaches on ground around walk-in freezer.
Floor soiled/has accumulation of debris. Observed spilled yellow substance on floor in corner by garage door. Observed spilled ice cream on floor by back door Observed floor at front counter soiled by food debris. Person in charge had Employee start sweeping front counter.

Observed approximately 2 rodent droppings on floor near white deep freezers. Observed approximately 43 rodent droppings on floor near triple sink and left of chrome wire rack. Observed approximately 46 rodent droppings inside unused ice cream machines to the right of walk-in freezer.
Spray bottle containing toxic substance not labeled. Observed orange and clear Chemical spray bottle filled with blue liquid missing common name labeled. Person in charge added label.
Cove molding at floor/wall juncture broken/missing. Observed cove molding under triple sink water damaged and separated from wall.

Floor tiles/planks missing and/or in disrepair. Observed vinyl floating floor planks popping up around toilet in bathroom close to wall.
Food stored on floor. Observed 5 cases of frozen fruit pops on floor of walk-in freezer.
