Per operator, food products have been out of temperature for more than 24 hours.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed shredded cheese (51F - Cold Holding); Gouda (46F - Cold Holding);cooked pork (51 F - Cold Holding); ham (54F - Cold Holding). Per operator, food products have been out of temperature for more than 24 hours. A stop sale was issued.
Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed chlorine concentration over 200+. PIC diluted, second test 100ppm.

Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Observed freshly squeezed orange juice for grab and go missing label information. Operator placed cups inside the reach in cooler behind the front counter to no longer have as grab and go.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled shelves housing cans of soda and plastic water bottled at the front counter.
Reach-in cooler shelves with rust that has pitted the surface. Observed in the upright cold holding unit across of POS machine and under prep counter.

Observed cooked chicken (123F - Hot Holding); cooked beef (121F hot holding); Piccadillo (108F - Hot Holding); inside hot box at front counter. As per operator foods were stored less than 2 hours. Operator reheated all food to 165F and above.
Certified food manager unable to answer basic Food Code questions pertaining to safe operation of establishment. Operator did not know minimum cooking temperature of chicken. Provided operator with cooking chart handout.
