Employee put on one glove and touched soiled surface - raw beef burgers. Then employee took off glove and handled clean equipment and utensils, without washing hands. Reviewed proper hand washing and employee washed hands.
Food-contact surfaces sanitized with a quaternary ammonium solution not exposed to the sanitizer solution for at least 30 seconds. Do not use equipment/utensils not properly sanitized. Employee washed utensils and then submerged in quaternary solution for approximately 5 seconds. Reviewed proper procedures and employee rehashed items.
