Operator placed bags of ice on top of items.

Approximately 5 flying insects flying around and landing on cases of single service items and packets of chips in kitchen area behind door next to dishwashing machine area.

Observed : Reach in cooler corned beef (41-47F - Cold Holding); pastrami (41-45F - Cold Holding); chicken salad (41-46F - Cold Holding); cut tomatoes (41-45F - Cold Holding). Items were packed above fill line which resulted in the top layers being out of temperature. Items were out of temp for approximately 15minutes. Employee removed top layers and placed in walk in cooler to rapid cool.

Equipment and utensils not properly air-dried - wet nesting. Observed plastic containers wet nesting in preparation table next to steam table. Employee removed and stored containers in a manner that will allow proper air drying.
