Employee wiped face with hand and after without washing hands employee handled clean and sanitized dishes, discussed with operator employee washed hands.
Rodent activity present as evidenced by rodent droppings found. In employee break room approximately 25 rodent droppings on floor under employee locker. In employee break room approximately 20 rodent droppings under rolling storage rack with boxes of dishes. In employee break room approximately 5 rodent droppings along wall by door. In kitchen approximately 1 rodent droppings on floor in front of soda dispenser storage rack. In kitchen approximately 1 rodent droppings on floor in dry storage. Discussed with operator to clean and sanitize all areas.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In low boy reach in cooler cut lettuce (45F - Cold Holding); shredded cheese (49F - Cold Holding) per operator products not portioned or prepared today and held in unit for approximately 3 hours, discussed with operator who moved product to walk in cooler to rapid chill.

Observed employee used lower section of hand to wipe across nose while he sniffled then sprayed whip cream on beverage and serve; no hand wash. Employee washed hands.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At low boy reach in cooler: sliced tomatoes (45F - Cold Holding); cut lettuce (47F - Cold Holding) As per manager, items removed from the walk in cooler stored since 7:00am and stored in unit. Not prepped or portioned today. As per operator, items sat on cart for a few minutes prior to placing in cooler. Operator moved items to walk in cooler to quick chill.
