Person in charge moved sausage to proper storage levels.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed; missing vacuum breaker on non-chemical dispenser side of splitter at mop sink faucet. Missing vacuum breaker on both sides of splitter on hose bibb located on exterior of build near drive-thru lane speakers. Employee added vacuum breakers to all necessary fittings.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone stored on prep table near single service items at main to-go window. Person in charge removed cell phone.

Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. At cook line reach-in freezer, open bag of raw chicken stored over open bag of partially cooked sausage. Operator moved chicken to bottom shelf.
Handwash sink used for purposes other than handwashing. Handwash sink at drive thru area used as dump since as evidenced by ice in sink at arrival for inspection.
Duct tape used to repair nonfood-contact surface. At drink station near drive thru, duct tape used on front of reach-in cooler.
Floors, walls and/or ceilings soiled/has accumulation of debris. Floor at front fryers has grease build up. Floor by ice machine soiled. Floor in walk-in freezer has build up of black residue. In dining room, floor in cabinet under drink station has build up.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Sides and front of fryers have grease build up. Multiple reach-in cooler gaskets throughout kitchen soiled.
Single-service articles not stored inverted or protected from contamination. At drive thru area, coffee filters on top,shelf with no protection. Also, in storage area, open case of cups stored on floor.
Wet mop not stored in a manner to allow the mop to dry. Multiple wet mops not hanging or inverted to air dry.
