Ice scoop stored on soiled surface behind tea dispenser next to ice machine. Employee cleaned surface and placed scoop in dish area.
No paper towels or mechanical hand drying device provided at handwash sink. Observed paper towels missing at hand wash sink across from three compartment sink. Person in charge provided paper towels.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone on top shelf of prep table across from hamburger reach in freezer. Person in charge removed employee cell phone.
Missing drain plug at dumpster. Observed missing drain plug on dumpster located at rear of establishment.

Single-service articles not stored inverted or protected from contamination. Observed coffee filters stored in box of coffee not protected. Box of coffee was in dry storage area with no cover. Employee placed filters in plastic bag to cover properly.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice machine has black mold-like substance on side wall. Operator had employee clean.
Carbon dioxide/helium tanks not adequately secured. Two tanks at soda storage rack not secured. Operator had employee placed chain on tanks.
Floor area(s) covered with standing water. In dining area, operator stated a water line under the floor is leaking. Employee mopped water.
