Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler on cook line to right of grill, Canadian bacon (44F - Cold Holding). Operator stated it was put in cooler form walk-in cooler no more than 1 hour and put item back into walk-in cooler to bring temperature down. Also, in make table reach in cooler on cook line, 6 pans cut lettuce (50-52F - Cold Holding). Operator stated cut lettuce was left in cooler over night and cooler was turned off. Operator voluntarily discarded.
Toxic substance/chemical improperly stored. In back storage room, spray bottle of freezer and cooler cleanser stored in box of small paper to go bags. Operator removed spray bottle and discarded top bags on the pile.

Raw animal food stored over/not properly separated from ready-to-eat food. In walk-in cooler, open package of raw beef in container on top of box of ready to eat sliced American cheese. Manager moved container of raw beef to different area away from ready to eat food.
Toxic substance/chemical improperly stored. In storage room, open box of grill cleaner stored on top of box of brown paper to go bags. Manager moved chemicals to shelf away from single service items. Also, in storage room open box of drain cleaner stored on top of box of labels used to seal to go bags. Manager moved labels to different shelf with other single service items.
Handwash sink used for purposes other than handwashing. Handwash sink in back of kitchen next to ice bin filled with ice. Manager removed ice from sink.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink cup on front line table next to where to go food is packaged. Manager discarded drink.

Employee near front counter placed hand on mouth with bare hand and immediately handled single use cups without washing hands. Manager coached employee on proper hand washing procedures. Employee working in fryer area (chicken) touched glasses, nose and face with gloved hands and immediately began preparing burgers without washing hands. Manager coached employee on proper hand washing procedures.

On the main make line in middle of kitchen, container of cut tomatoes and container of cut lettuce held on time per manager not time marked. Manager states items removed from temperature control approximately five minutes prior. Manager marked items.
