Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed a splitter added to mop sink without back flow device on non chemical side of splitter. Manager was able to add back flow during inspection.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at employee hand sink located near walk in freezer. Employee provided paper towels for station.
Light shield damaged/in disrepair. Observed cracked light shield located in walk in freezer.

Floor soiled/has accumulation of debris. Along wall in dry storage/ mop sink area. Along wall under rack by walk in freezer door. In walk in freezer.
Ice buildup in reach-in freezer and/or walk-in freezer. In small reach in freezer at end of cook line near grill tops.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Interior of cabinets under French fry warmer on cook line. Interior of cabinet at end of make line across from drink station. Underside of frozen French fry dispenser next to fryers.

Toxic substance/chemical improperly stored. Chemical spray bottle stored on shelf over clean utensils on the shelf opposite the three compartment and hand wash sink. Operator relocated the chemical spray bottle.

Ice buildup in reach-in freezer and/or walk-in freezer. Build up of ice around walk in freezer door and seal.
Single-service articles improperly stored. Single service happy meal carry out containers stored in a box on the floor by the drive through window. Operator has employee relocated the to go boxes.
Wiping cloth chlorine sanitizing solution not at proper minimum strength. Sanitizer solution at the end of the main cook line closest to the office area at 0ppm chlorine. Operator had employee replace sanitizer solution to 100ppm.
