Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink, vacuum breaker installed at beginning of threaded faucet with splitter added. No vacuum breaker present on non-chemical side of splitter.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink by second drive thru window. Employee replaced towels. Also, no soap at hand wash sink at first drive thru window.
Equipment in poor repair. Microwave by fry reach in freezer missing handle.
Ice buildup in reach-in freezer and/or walk-in freezer. Excessive ice build up in wall mounted reach in freezer by office.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Under-plating of both soda machines in kitchen and frappe machine are soiled. Employee began to clean during inspection. Cup lid holders at frappe machine soiled. Employee began to clean during inspection.
Wiping cloth chlorine sanitizing solution not at proper minimum strength. Sanitizer bucket near dish room tested at 0ppm chlorine. Employee remade solution to 100 ppm chlorine.
