Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Men's restroom door not closing tightly creating a gap. Manager states door is on scheduled to be repaired.

Ceiling/ceiling tiles/vents/walls/floors soiled with accumulated food debris, grease, dust, or mold-like substance. Floor under fryers on right side soiled. Area on both sides of fryer with food debris.
Garbage can located outside has no lid or lid open/broken. Dumpster has right side lid broken off and laying on the ground. Left lid in disrepair.
