Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed reach in cooler on line across fryer with ambient temperature of 58F. Operator had repairman come to check on cooler. The following food stored inside more than 4 hours prior to inspection with no indication when cooler stopped functioning: cheese slices (58F - Cold Holding); bacon (58F - Cold Holding); butter sticks (58F - Cold Holding); ham (58F - Cold Holding)
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler on line across fryer with ambient temperature of 58F. Operator had repairman come to check on cooler. The following food stored inside more than 4 hours prior to inspection with no indication when cooler stopped functioning: cheese slices (58F - Cold Holding); bacon (58F - Cold Holding); butter sticks (58F - Cold Holding); ham (58F - Cold Holding). Prior to end of inspection, operator fixed reach in cooler; breaker was tripped. Final ambient read: 43F.

Employee switched from working with raw food to ready-to-eat food without washing hands. Observed employee put on blue gloves, placed raw beef patty on flat top, change gloves and continue process by grabbing raw onions placed on flat top with no hand wash in between use of gloves. Observed employee remove gloves and wash hands properly before continuing. Coached employee on proper hand wash techniques between uses.
Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. Observed employee put on blue gloves, placed raw beef patty on flat top, change gloves and continue process by grabbing raw onions placed on flat top with no hand wash in between use of gloves.
Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Observed on approved plans hand wash sink removed from ware wash area. Observed pipes in wall, and drain capped off.
No soap provided at handwash sink. Observed no soap in men's restroom in stall.
Employee with ineffective hair restraint while engaging in food preparation. Observed employee with hat on with a long ponytail swinging from side to side and not effective in restraining hair properly while preparing breakfast sandwiches. coached employee and employee restrained hair in bun.
Equipment and utensils not properly air-dried - wet nesting. Observed mop not hanging up right, left on floor of mop sink. Coached manager and they hung mop up correctly.
Floor area(s) covered with standing water. Observed standing water behind soda syrup dispenser in back of kitchen.
Garbage can located outside lid in disrepair. Observed dumpster lid damaged.

Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. At the time of the inspection observed employee personal food stored in the walk in cooler. Manager removed it.
Ice buildup in reach-in freezer and/or walk-in freezer. At the time of the inspection observed build up ice in walk in cooler.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. At the time of the inspection observed soiled fan cover in the reach in freezer located by the cook line. Observed reach in cooler fan cover soiled located by the kitchen. Observed fryer soiled located next to the reach in freezer.
