Ceiling/ceiling tile shows damage or is in disrepair. Several gaps and/or holes in ceiling tiles in back of kitchen above fountain sodas.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. One employee working at cookline and handling food with several bracelets on wrist.

Ice buildup in reach-in freezer and/or walk-in freezer. Buildup of ice around walk-in freezer door.
Missing drain plug at dumpster. Drain plug missing from both dumpsters.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Area around nozzles of fountain machine heavily soiled with mold-like substance.
Old food stuck to clean dishware/utensils. Old cheese stuck to interior of plastic pan on clean dish rack. Manager placed the pan in the sink to be washed, rinsed, and sanitized during this inspection.
Plumbing system in disrepair. 1. One handwash sink faucet is not functioning in kitchen near cookline. Manager stated a work order has already been placed and it will be repaired soon. 2. Dripping line below fountain machine in lobby.
Unclean building components, attachments or fixtures. Floor inside walk-in cooler soiled with food debris under shelving.
