Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed person in charge unable to provide written procedure in the event of vomiting and diarrhea. Inspector provided form.

Equipment and utensils not properly air-dried - wet nesting. Observed clear plastic containers stacked on top of each other with visible signs of moisture and unable to air dry stored on rack next to three compartment sink.
No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign at hand wash sink at end of cook line and in dry storage area.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled gaskets on reach in cooler and reach in freezer doors on cook line.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored not inverted in mop sink.
Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth stored on reach in cooler at end of cook line and not stored in sanitizing solution between uses.

No paper towels or mechanical hand drying device provided at handwash sink. No soap or paper towels at hand wash station located near front register/drive thru. Operator supplied both.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. One employee hired under 60 days was not informed of responsibilities. Operator stated employee in question will be coming in today. Employee will sign then.
Floor soiled/has accumulation of debris. Floor under flat top grill has build up of grease.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At hot holding unit in kitchen by fryers, hash browns (115F - Hot Holding, retemped after 5 mins, 173F). Manager stated hash browns were made 15 mins prior. Manager dropped hash browns in fryer, 173F.
Toxic substance/chemical improperly stored. In chemical storage closet next to drink machine in customer lobby, cleaning chemicals stored next to paper towels. Manager removed paper towels.
Handwash sink used for purposes other than handwashing. Hand wash station by triple sink has dirty dishes in sink. Employee removed dishes. Hand wash station by smoothie maker has food debris in sink. Employee removed food debris.
No paper towels or mechanical hand drying device provided at handwash sink. At hand wash station behind front register, no paper towels. Manager added paper towels.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee phone above prep table in kitchen. Employee removed phone.
Equipment in poor repair. Reach in freezer door by fryers in kitchen hanging off hinges. All TCS foods within temp. Operator moved frozen foods into walk in freezer until repaired.
Food stored on floor. Boxed in bag sodas on ground. Manager removed from ground.
