Cooked poultry not reaching a minimum internal temperature of 165 degrees Fahrenheit. At cookline: chicken (151F - Cooking). Manager had the employee continue to cook the chicken, then 206F.
Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee at cookline pulled up pants with gloved hands, then proceeded to handle Canadian bacon with the same gloved hands without first washing hands and putting on new gloves. Manager had the employee wash hands during this inspection.

Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Three compartment sink quaternary over 500ppm. The manager diluted the sanitizer with water, then 300ppm. Inspector instructed the manager to have all dishes rewashed, rinsed, and sanitized that have been washed today using the three-compartment sink.
Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. Manager stated no employees at this location are trained to use the dish machine. The area supervisor arrived during this inspection and instructed the manager on how to use the dish machine.
Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Employee with artificial and/or painted nails scooped food without wearing gloves near front counter. The employee put on gloves during this inspection.
Handwash sink not accessible for employee use at all times. 1. Handwash sink beside three-compartment sink blocked by trash can. 2. Handwash sink at end of cookline blocked by handled dust pan and trash can. Both handwash sinks were unblocked during this inspection.
Dead roaches on premises. One dead roach on floor behind front counter. An employee swept the dead roach during this inspection. The establishment is currently on open warning for this violation.
Ice buildup in reach-in freezer and/or walk-in freezer. Buildup of ice on floor inside walk-in freezer.

The dish machine is not set up and the manager stated no employees at this location know how to operate the dish machine. The quaternary sanitizer at the three-compartment sink is not dispensing properly. Inspector had the manager call ecolab to service the dish machine during this inspection.
Employee with no beard guard/restraint while engaging in food preparation. Two employees with beards working at cookline without wearing beard restraints. The manager had both employees put on beard guards during this inspection.
