Observed employee reach into pocket to answer phone, answered phone touching side of head and worked with bagging drive through food during inspection
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels available at hand wash sink in kitchen. Operator had employee replace paper towel during inspection.
Floor soiled/has accumulation of debris. Observed togo cup lids/trash around equipment/table legs in front kitchen at time of inspection.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up in walk in freezer on door and floor under fan units at time of inspection

Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance on ice dispenser chute in drive thru area. Operator had employees disassemble and clean during inspection.
Floor soiled/has accumulation of debris. Observed soiled floor around equipment back by dish room area during inspection. Operator had employee start cleaning during inspection
Food stored on floor. Observed opened case of chicken on floor of freezer. Operator moved case during inspection

Missing drain plug at dumpster.
Wet mop not stored in a manner to allow the mop to dry. Mop in maintenance closet by bathrooms stored in mop bucket. Operator placed mop on mop rack to dry during inspection.
