Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee at front counter wiped hands on front of her shirt, then proceeded to scoop fries without washing hands. Manager had the employee stop and wash hands before continuing to work with food.
Food-contact surface soiled with food debris, mold-like substance or slime. Nozzles and area around nozzles of fountain machine in lobby soiled with mold-like and/or lime-like substances. An employee began cleaning the fountain machine during this inspection.
Dead roaches on premises. 1. Two dead roaches inside hole in wall in back of kitchen. 2. One dead roach on floor in kitchen, near soda rack. An employee cleaned both areas during this inspection.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee jacket and keys stored on shelf directly above condiment packets in drive through area. The employee removed all personal items from the shelf during this inspection.

Raw animal food stored over/not properly separated from ready-to-eat food. Pan containing portions of raw hamburger meat stored on shelf directly above caramel frappe mix inside wall-in cooler. Manager removed raw hamburger meat from shelf during this inspection.
Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tile hanging with gap into ceiling near drive through window.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Area around nozzles at fountain machine in lobby soiled with mold-like substance. Employee began cleaning area around nozzles during this inspection.
Unclean building components, attachments or fixtures. 1. Several dusty air vents in kitchen near cookline. 2. Several dusty ceiling tiles in kitchen at cookline. 3. Floor under and around shelving and equipment throughout kitchen soiled with food debris. Manager had employee clean floor around drain behind front counter during this inspection.

Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloth stored on prep table near cookline. Manager placed wiping cloth back into sanitizer bucket during this inspection.
