Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior deflector plate of ice machine permanently stained black.
Equipment and utensils not properly air-dried - wet nesting. On rack above three compartment sink, multiple plastic pans stacked before properly air dying.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets soiled/torn in kitchen. Operator stated order has already been placed for all new gaskets. Also, on underside near nozzles soiled with residue on ice cream, frappe and juice machines.
Reach-in cooler interior/shelves have accumulation of soil residues. Interior of reach in cooler in drive thru soiled with ice cream mix. Operator had employee clean cooler during inspection.
A return visit. The problems from the previous inspection had been dealt with.

Employee on cook line cracked raw shelled eggs with gloved hands, removed gloves, and immediately handled clean tray for cooked products without washing hands. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.
Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee in kitchen placed stickers in her mouth, handled single use food containers for hash browns, placed hash browns in containers, removed stickers from mouth and placed on stickers. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.

Dead roaches on premises. One dead roach on floor in utility closet near restrooms. Dead roach discarded.
Ice buildup in reach-in freezer and/or walk-in freezer. Walk in freezer. Manager states work order has been placed.
Missing drain plug at dumpster. Missing for both blue dumpsters outside establishment. Manager states calling dumpster company today.
