Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee at grill station putting on blue gloves and handling raw steak and placing on grill. Employee then removed blue gloves by taking on opposite hand to remove and slide gloved hand and repeating same process with other hand and discarded in trash. Employee did not wash hands afterwards and then went to handling clean equipment. Spoke to manager on duty and informed me that Mc Donald has new procedure for past year since December to no longer shake the blue gloves off into the trash but instead using other hand to grab and slide gloves off and discarded. Manager said will review this with his supervisor
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Missing back flow preventer at non-chemical side of splitter at mop sink area. Manager put back flow preventer on
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. At handwashing sink near wear washing area. Manager provided paper towel
