Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Upon inspection, observed mop sink faucet in disrepair, missing bell housing for vacuum breaker.
No soap provided at handwash sink. Upon inspection, observed no soap available at handwashing sink near grill area at establishment. Manager stated dispenser batteries need replacing.

Wiping cloth chlorine sanitizing solution not at proper minimum strength. Upon inspection, observed reading of 0 ppm chlorine in sanitizer bucket. Bucket was just made by employee. Manager stirred bucket during inspection and 100 ppm reading was observed.
Nothing was found. The inspector recorded no violations at all.

Equipment drain line draining into handwash sink. Portable air conditioning unit output water is draining into handwashing sink by fry station.
Handwash sink not accessible for employee use at all times. Handwashing sink in kitchen by fry station is inaccessible due to portable air conditioning unit being attached to faucet.
No paper towels or mechanical hand drying device provided at handwash sink. Handwashing sink on grill side of kitchen is empty. Operator refilled paper towels during inspection.
Equipment in poor repair. Air conditioning units are not functioning. Operator has set up large portable air conditioning units to offset heat build up and keep air flow. HVAC technician present during inspection verifying parts needed to order before starting repairs.
Plumbing system in disrepair. Handwashing sink handle is broken on grill side of kitchen.

Equipment and utensils not properly air-dried - wet nesting. Upon inspection, observed storage containers on dish rack wet nesting. Manager removed from rack and placed containers back in dishwashing area to be washed.
