Observed employee touch hair and ear and went to resume working with food. Observed manager intercepting employee and had employee wash hands

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dish machine with low reading on chlorine 25 ppm. Observed manager changing pellets and re tested at 50 ppm.

Presence of insects, rodents, or other pests. Observed 10 dead knats, 1 live ant, and 2 dead flies on premise while open, operating, and selling of food. Location for knats, in drive thru area in reach in self serve stations. Live ant crawling on floor next to door leading into dining room area by back drive thru window. 2 dead flies on top of hot water tank. Observed Managers cleaning and sanitizing immediately getting rid of dead insects.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance on gaskets of icee machine near back drive thru window.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed multiple employees with jewelry on while handling food.
Employee with no hair restraint while engaging in food preparation. Observed employee at hash brown/fry station setting up hash browns with out a hair restraint. Observed manager coaching employee and employee putting on hat.
Floor not cleaned when the least amount of food is exposed. Observed in storage rack area from the slide rail to the wall, accumulation of debris.
Reach-in cooler interior/shelves have accumulation of soil residues. Observed food debris in reach in at meat station.
