Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Employee at grill unable to give minimum cooking temperature requirements for hamburger meat. Inspector educated employee during this inspection.

Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Screwdriver stored on top of reach-in cooler beside grill. Manager removed the screwdriver from the area during this inspection.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee at drive through window scooped ice with a takeout cup. Employee's bare hand came into contact with ice.
Food-contact surface soiled with food debris, mold-like substance or slime. Fountain machine nozzles in customer dining area soiled with mold-like substance.
No soap provided at handwash sink. Soap dispenser empty at small handwash sink at front counter.
Spray bottle containing toxic substance not labeled. Spray bottle containing degreaser missing label on floor near mop sink. Manager added proper label to bottle of degreaser during this inspection.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Ice bucket stored not inverted on top of ice machine. Manager inverted bucket during this inspection.
Floor soiled/has accumulation of debris. 1. Floor inside walk-in freezer soiled with food debris. 2. Floor near water heater and fountain soda boxes in back of kitchen soiled with dirt.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup around door inside walk-in freezer.
Water leaking from pipe and/or faucet/handle. Pipe leaking up in ceiling in back of kitchen. Manager stated the pipe was coming from the air conditioner and maintenance has been made aware of the problem.
A return visit. The problems from the previous inspection had been dealt with.

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee entered the kitchen from the dining area and began packing hash browns without washing hands. Manager had employee stop and wash hands before continuing to work at cookline.
Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. One fly on ice bucket above ice machine in kitchen.
Rodent activity present as evidenced by rodent droppings found. 28 rodent droppings on shelf beside sleeves of takeout cup lids and straws below fountain machine in dining area. Manager stated pest control set traps and caught a mouse a few days prior.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Backflow prevention device missing at black hose connection in dumpster enclosure behind building. Backflow prevention device missing at hose connections after splitter in mop sink near three-compartment sink.
Plumbing system in disrepair. Built-in backflow preventer at mop sink in kitchen missing parts.
Wiping cloth chlorine sanitizing solution not at proper minimum strength. Wiping cloth bucket at front counter chlorine 0ppm. Manager had employee empty and refill sanitizer bucket, then 100ppm.
