Soap dispenser at handwash sink not working/unable to dispense soap. Soap dispenser at hand wash sink by partition door in kitchen not working. Person in charge fixed during inspection.
Bathroom fixtures in poor repair. Observed toilet paper dispenser in men's restroom in poor repair. Per person in charge clip is broken so front of enclosure hangs open exposing internal components of dispenser. Person in charge provided temporary fix during inspection until new dispenser can be ordered.
Dead roaches on premises. Observed one dead roach on the floor near sanitizer bucket storage near hand wash sink by partition door in kitchen. Person in charge discarded.

Live, small flying insects found. Observed approximately 5 flying insects around self service soda dispensers in dining area. Observed approximately 10 flying insects in back kitchen near drain by back door by CO2 tanks. Person in charge began killing flying insects and cleaning areas.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed dried onions rehydrated at 8:46 am at 74F at 1:25pm. Onions stored in covered insulated plastic containers.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Butter in insulated container in reach in cooler drawer at front line at 44-46F. Person in charge moved to walk in cooler to rapidly chill. Milk at coffee station at 50F. Person in charge states has been in unit for 3 hours, moved to walk in cooler to rapidly chill.
Time/temperature control for safety food prepared from/mixed with ingredient(s) at ambient temperature not cooled to 41 degrees Fahrenheit within 4 hours. Observed dried onions rehydrated at 8:46 am at 74F at 1:25pm. Onions stored in covered insulated plastic containers. Per person in charge utilizes cold water. See stop sale.
