Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the left dining area soda machine has soiled soda ports. This unit is not in service due to the ice machine is not working.
Handwash sink not accessible for employee use at all times. Observed : the drive thru hand wash sink was blocked with a rolling cart. The cart was moved.
Records/documents for required employee training do not contain all of the required information. Observed : Employee Training records are not signed off by a manager. The manager started signing the forms from the McDonalds training.

Carbon dioxide/helium tanks not adequately secured. Observed : the back up co2 tank was not secured. The chains was added.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed : 1 cell phone stored at the back prep table near the office. The phone was moved to the office.
Exterior door has a gap at the threshold that opens to the outside. Observed : the walk-in freezer has an exterior exit/entrance, this has a gap along the whole bottom edge of the door.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed : the walk-in freezer has an accumulation of ice building up on the floor. A hammer was being used to break up the ice.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : the cabinet under the dining area soda machine has an accumulation of trash.
Single-service articles improperly stored. Observed : single serve cups stored on the floor, front line corner.
Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed : the cook line hand wash sink was 1\2 full of water, not draining.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed bulk butter 66° on the prep table near the office. This was taken out around 10 am per the staff. This was placed in the walk-in freezer and was at 55° 9 minutes later.
Toxic substance/chemical improperly stored. Observed restroom spray and glass cleaner spray stored on the drive thru to go condiment rack. These were moved to cleaning.
Floor tiles missing and/or in disrepair. Observed: base cove tiles not attached to the wall, across from the 3 compartment sink area.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed: in the top reach-in freezer on the fry line, an ice pad has formed.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed: the top of the shake/soft serve machine has an accumulation of debris.
Water leaking from pipe and/or faucet/handle. Observed the drain in the dining area drink machine drains into a bucket because the drain pipe is compromised.
Worn, torn and/or soiled floors/carpeting. Observed the floor in the walk-in freezer has a pad of ice covering most of the floor.

Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed: the sliced cheese on the cook line lacked a time mark. An adjusted time sticker was added.
Toxic substance/chemical improperly stored. Observed combo cleaner stored on the Catina machine. The container was placed below food and clean dishes.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: both juice nozzles have build up in the tubes.
Bulk milk dispenser with dispensing tube longer than one inch protruding from the chilled dispensing head. Observed: the bulk milk dispenser by the drive thru, milk tube over 1 inch and not at a 45° angle.
Floor area(s) covered with standing water. Observed standing water on the floor in front of the carbonator.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed: inside the entrance to the walk-in freezer, an accumulation of ice on the floor.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed: the ice scoop for the drive thru area, handle was touching the ice.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced cheese in the fry line reach-in cooler, 47F. This was moved to the walk-in freezer and then after it reached 43F it was moved to the walk-in cooler. This pan of cheese was prepped this morning.
Toxic substance/chemical improperly stored. Observed: toaster cleaning chemical stored over reach in with beef on the cook line. ALSO, spray chemical stored at the drive thru on the portion rack. These were both moved to cleaning.
