Handwash sink used for purposes other than handwashing. Observed grease like substance in hand washing sink towards back of kitchen, operator cleaned.

Drain cover(s) missing. Observed drain cover missing under 3-compartment sink and drain cover missing over drain located under cabinet under soda dispenser in dining room.

Worn, torn and/or soiled floors/carpeting. Observed standing water on floor in back of kitchen in the hallway, in front of walk in cooler and in crew room.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee not wearing gloves scooping fries as some of the fries were falling on the employees hand, operator stopped and addressed the employee. Employee washed hands and put gloves on.
Toxic substance/chemical improperly stored. Observed 3 bottles of chemical hanging on the shelf by single service cups in storage area, operator moved.
Drain cover(s) missing. Observed drain cap missing under 3 compartment sink.
Employee eating in a food preparation or other restricted area. Observed an employee eating an apple through a food prep area.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink has a4way splitter and not two outlets are covered with vacuum breaker.
Floor soiled/has accumulation of debris. Grout receded and old debris stuck in grout lines throughout the kitchen area.
Floor tiles missing and/or in disrepair. Observed floor tiles broken out at back dry storage.
Old labels stuck to food containers after cleaning. Observed one plastic container in clean dish storage with partial label still on outside. Manager removed to dishwash.
Single-service articles improperly stored. Observed cups at drive through not protected from above contamination. Caps missing from cup dispenser. Manager placed plastic wrap over opening.
