No soap provided at handwash sink. Observed no soap at the hand washing sink in the men's bathroom, operator had it changed out.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish machine is not working at all, they are using 3-compartment sink, operator stated will submit a work order.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed a cell phone on a prep table towards front counter, operator moved it.
Employee with no beard guard/restraint while engaging in food preparation. Observed one of the employees without a beard guard, employee put one on.

Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Observed employee on the make line removing empty trays in warmers and replacing with food filled trays. Operator had employee wash hands and add gloves
Employee with long fingernails working with exposed food, single-service items or clean equipment and utensils. Observed employees dipping ice serving fries into cups and handing bagged food and straws and napkins at drive through. Manager had employee wash hands and add gloves.
No soap provided at handwash sink. At back drive through window area. Operator replaced soap.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Over dispensing chlorine. Found at 200+ strip turned black. Operator put in service call will use 3 bay until corrected.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Manager had employee wash hands and add gloves.
Wall soiled with accumulated grease, food debris, and/or dust. Observed black growth up to 1 foot high on wall behind dry storage racks.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cookline reach on cooler had been accidentally turned off observed raw shell eggs (2flatts (57F - Cold Holding); egg omlette 10 sleeves (57F - Cold Holding); rehydrated onions 1pint (57F); liquid eggs 4.4lbs (57F - Cold Holding) see stop sale. Manager discarded all and discontinued use of cooler until it is turned on long enough to be sure it can maintain foods at 41F of colder.. Note: early morning temperatures of cooler recorded at 36F on McDonalds HACCP log.
Trash receptacles not provided where needed in establishment. No trash can beside handsink at ice machine area.
