No soap provided at handwash sink. Observed : at the front line hand wash sink, no sign, no soap. ALSO, the hand wash sink at the tea/lemonade station lacked a sign, soap and paper towel.
Bulk milk dispenser with dispensing tube longer than one inch protruding from the chilled dispensing head. Observed : the milk dispensing tube was over 1" long, and was not cut at an angle.
Wash/Rinse/Sanitizing solution not maintained clean. Observed : the 3 compartment sink sanitizer sink water was soiled. This was dumped to make a fresh solution.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed : liquid eggs in a carton, on the cook line 49°. This was placed open in the walk-in freezer and achieved 38° in 11 minutes.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : soda nozzles in the dining room have build up, on the right Fruit punch, Diet Coke and orange.
Handwash sink used for purposes other than handwashing. Observed : in the hand wash sink by the 3 compartment sink, a pair of tongs. These were removed.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : an accumulation of splash up behind the OJ dispenser.
Worn, torn and/or soiled floors/carpeting. Observed : food and debris accumulation in multiple corners of the kitchen, and cash accepting drive thru area.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the Latte machine dispensing heats has an accumulation of dried splash up. ALSO, the Orange juice machine nozzles have an accumulation of dried juice in the nozzle. The dining area soda nozzles have build up.
Bulk milk dispenser with dispensing tube longer than one inch protruding from the chilled dispensing head. Observed: the milk dispenser tube was over 1 inch. The tube was not cut at a 45° angle. This was cut to 1 inch and at an angle.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed at the dining area soda machines, the area behind each machine has an accumulation of soil. ALSO, the cabinet under the soda machines has an accumulation of spills. The drink machines in the kitchen have dried splash up behind the drink dispensers nozzles.

Floor tiles missing and/or in disrepair. Observed: floor tiles on the cook line are broken.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed: in the walk-in cooler, the top half of the cooling fins are solid frozen over. Temperature are 42-43F.
Stored food not covered. Observed: time/temperature control for safety food in the walk-in freezer, frozen and uncovered. Items were then wrapped.
Vending machine exterior soiled. Observed: the area behind the water nozzle on the left soda machine is soiled. ALSO, the Freezzee machine exterior nozzles are soiled. This item
Wiping cloth sanitizing solution stored on the floor. Observed at the end of the cook line, sanitizing bucket was on the floor. This was placed in a cart.
