Handwash sink not accessible for employee use at all times. Observed at handwashing sink next to ice machine - sink blocked by cart, operator removed and gave access to sink.

Prep station at hot holding unit- observed employee wipe food debris off prep table, put food debris in trash, continue to unwrap single service sandwich wrap and place on station without washing hands or changing gloves.
