Employee at cookline touched hat, handled a wiping cloth, then handled food containers at the cookline without washing hands. Manager had the employee stop and wash hands before continuing to work at the cookline.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. 1. Backflow prevention device missing at spigot on wall inside compactor enclosure. Manager added a Backflow prevention device to the spigot during this inspection. 2. Backflow prevention device missing after splitter added to mop sink. Manager added a Backflow prevention device to the splitter during this inspection.
Cardboard used to line nonfood-contact shelves. Shelving in dry storage area lined with cardboard.
Equipment and utensils not properly air-dried - wet nesting. Wet nested clear plastic pans on clean dish rack. Manager restacked the pans to allow them to dry during this inspection.

Manager stated employees placed milk in cooler two hours prior to temperature being taken. Manager voluntarily discarded milk.

Food storage container/container lid cracked or broken. Crack in plastic lid. Manager removed lid from use.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Mold like substance on the underside of the non food contact surface of the nozzles on the frosty and latte dispensing machine at server station.
Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloth on serving surface of drink machine. Manager removed Wet wiping cloth

Toxic substance/chemical improperly stored. Observed multiple chemical spray bottles hanging on rack all facing paper towels used at hand wash sink. Manager removed chemical spray bottles.
Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Observed hand wash sink removed next to ice machine at server station.
Bulk milk dispenser with dispensing tube not cut at an angle. Observed milk dispenser not cut an an angle at the server station.
Toilet/urinal not flushing/functioning properly. Observed one of the two urinal out of service.
