In walk-in cooler, rehydrated onions (57, 60, 61F - Cold Holding). Tag on containers showed item prepared 4:05 hours earlier. Wet wiping cloth not stored in sanitizing solution between uses. At cook line, wet towel store on counter.

Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Cook on grill wiped hands on apron (put hand in pocket of apron) and immediately began to open precooked eggs without washing hands. Manager coached employee on proper hand washing procedures.
Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. One live fly observed in back dish area.
