Certified Food Manager or person in charge failed to notify the division of employee diagnosed with Norovirus, Hepatitis A, Shigella spp., Shiga toxin-producing Escherichia coli, Typhoid fever (caused by Salmonella Typhi) or Salmonella (nontyphoidal). Observed establishment lacking of employees health reporting agreement, as well employees lacking ok knowledge as to report the person in charge. Manager was provided with form.

Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. During the Licensing inspection observed a Hand Sink missing next to the three compartment sink at kitchen area. Hand sink is listed on the Plans submitted.
Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Operator printed and posted poster on site.
