Employee at cookline stated the sliced ham was placed inside the unit from the walk-in cooler approximately one hour prior and placed the ham into the walk-in cooler to cool to 41F. Inside refrigerated drawers below grill: country ham (55F - Cold Holding); cubed ham (48F - Cold Holding). Employee at cookline stated both items were placed inside the unit from the walk-in cooler approximately one hour prior and added ice to both items to cool to 41F. sliced ham (51F 8:50, 37F 10:00 - Cooling); country ham (55F 8:50, 42F 9:45 - Cooling); cubed ham (48F 8:50, 41F 10:00 - Cooling)

Employee switched from working with raw food to ready-to-eat food without washing hands. Employee at cookline cracked a raw shell egg with gloved hand, then touched a fully cooked omelette with same gloved hand without washing hands or changing gloves. Manager had employee stop and wash hands before continuing to prepare foods.
Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee touched hair and hat with gloved hands, then proceeded to handle raw hamburger meat without washing hands or changing gloves. Manager had employee wash hands and put in new gloves before continuing to work at cookline.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Inside reach-in cooler drawers below grill: diced tomatoes (53F - Cold Holding); sausage (48F - Cold Holding); cheese (50F - Cold Holding); sausage (46F - Cold Holding). Manager stated all items were placed into unit from walk-in cooler two hours prior and began placing items into freezer to cool to 41F.
