Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed cheese pizza held on time control without time mark. Employee placed time mark on pizza.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter added to mop sink without back flow device added to non chemical side of splitter.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed soda gun holders in bar area soiled with mold like substance. Employee cleaned during inspection.

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee came into prep area from outside, put on gloves then began to handle clean equipment without washing hands first.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. sliced tomatoes (49F - Cold Holding)in over filled container in top section of reach in cooler on make line, per manager top section just went through a thawing cycle that took no more then 2 hours, tomatoes moved to walk in cooler. Ricotta (53F - Cold Holding) sitting above ice bath on make line, per manager ricotta has been there for no more than a few hours, ricotta moved to walk in cooler.
Handwash sink not accessible for employee use at all times. Hand wash station in back prep area blocked by large vegetable washer/spiner, machine moved.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No chlorine test strips.
No paper towels or mechanical hand drying device provided at handwash sink. St hand wash station in back prep area, operator corrected during inspection.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Top interior of ice bin of soda machine by office has a build up of mold like substance, manager began to clean during inspection.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. shredded cheese (46F, 45F- Cold Holding); pizza sauce (46F, 45F - Cold Holding); sausage (45F,46F - Cold Holding); cut tomatoes (45F - Cold Holding); sliced (46F,46F - Cold Holding) in top section of reach in coolers on cook line across from pizza oven, per manager lids have been left off for approximately 4 hours, all items moved to walk in freezer, all items retemp at or below 41F.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. marinara (64F - Cooling, after 45 minutes 63F) in very large plastic covered container in walk in cooler, lid removed.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. diced tomatoes (47F - Cooling, 47F after 30 minutes ) in top section of reach in cooler next to flat top grill on cook line, per manager items where placed in cooler approximately 1 hour prior, spoke with other manager about adjusting temperature of cooler because of its placement on cook line.

Raw fruits/vegetables not washed prior to preparation. Employee cutting heads of romain in prep area before cutting them, employee stopped and began to wash the remaining heads of romaine.

Toxic substance/chemical improperly stored. Chemical spray bottle stored on shelf with clean containers over the three compartment sink. Operator removed the chemical spray bottle.
Employee with no beard guard/restraint while engaging in food preparation. At time of inspection two food handling employees with no beard guards working on the main cook line.
Equipment and utensils not properly air-dried - wet nesting. On the storage shelf opposite the rear exterior door, clean plastic containers stacked while wet. Operator instructed employee to unstack and rewash the containers.
Hood filter missing from automatic fire suppression/exhaust system. Hood filters on the main cook line over the flat top grill removed at time of inspection. Establishment currently cleaning the hood filters.
Old labels stuck to food containers after cleaning. On the storage shelf opposite the rear exterior door, clean containers with old labels attached. Operator instructed employee to remove the labels and rewash the containers.
Utensils in poor condition. Wooden pizza paddle in use on the main cook line has a chip in the wood at the end of the paddle. Operator instructed employee to use a new paddle.
