Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Wings in reach in cooler on cook line at 59F. Employee states wings were left out for orders for about 10 minutes. Wings moved to freezer to quickly chill and wings were at 41F approximately 20 minutes later.
Drain cover(s) missing. Drain cover missing under bar triple sink.
Interior of refrigerator or freezer in disrepair/has exposed insulation. Interior two left doors of flip top reach in cooler have panels missing, exposing insulation.

Per person in charge, all products left in cooler overnight. Also, ham (46F - Cold Holding); cheese (48F/46F - Cold Holding) in small reach in service cooler in kitchen. Per person in charge, both products have been in same cooler overnight.

At beginning of inspection, refrigeration repair company was leaving, manager called company to return and repair person arrived during inspection. Also, wings in reach in cooler by pizza oven at 48F. Person in charge states wings were left out on the counter for approximately one hour. Wings moved to walk in freezer and 30 minutes later temperature at 29F. Also, ham (46F - Cold Holding); cheese (48F/46F - Cold Holding) in small reach in service cooler in kitchen. Per person in charge, both products have been in same cooler overnight.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior sides of bar ice machine has black mold-like substance.
Floor/wall/vent/wall soiled/has accumulation of debris. Floor at right corner under dishmachine and under equipment in bar is souled. Wall under dishmachine is soiled. Fan guards in walk in cooler have dust/debris.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of two door oven has accumulation of debris.
Interior of refrigerator or freezer in disrepair/has exposed insulation. On middle flip top reach in cooler, two doors on right have interior panel missing with full door of insulation exposed.
Single-service articles not stored inverted or protected from contamination. Take out drink lids in container at soda machine are stored with interior portion up. Manager inverted all lids. Also, case of straws on floor in dry storage room.

Employee states brought tomatoes from walk in cooler at approximately 12:00. Temperature taken at 3:07. Tomatoes moved to walk in freezer and temperature taken approximately 15 minutes later at 38F.
